Ayer terminé Pediatría y el lunes comienzo mi última rotación.
¡A nada de graduarme!
Pero ya llegaremos ahí...
Yesterday I finished pediatrics, and on Monday I start my last rotation.I’m so close to graduating! But we will get there...
Pediatría
Hace nueve semanas, comencé mi rotación en Pediatría.
Nueve semanas peculiares, a tal punto, que esta última semana ni pude abrir Hive.
¡Una locura!
Me tocó terminar en el Cuerpo de Guardia del Hospital Marfán y es alarmante, la situación en nuestra ciudad.
El hospital va a ser reconvertido en Instituto Nacional de Implante Coclear. En medio de la reconversión, nos vimos envueltos nosotros.
La sala de Pediatría, donde debíamos rotar, fue cerrada (falta de personal, la reconversión, falta de insumos, todo sumó para esta decisión)
Las condiciones de la guardia eran horribles.
Tenía que recorrer todo el hospital para buscar un colchón. Era la única manera de poder descansar unas horas en la propia consulta.
De lo contrario, me tocaba dormir arriba de un buró (sí, como en [Cirugía]() )
El servicio se limitó, solo para pacientes muy graves, basados en unos criterios de ingresos absurdos.
El resto de pacientes, por la realidad del país, se remitían a otras instituciones (bastante lejanas) o se les indicaba vigilancia por el área de salud.
¡Un desastre!
Cada día ser médico en Cuba es más difícil. Pero ser padre es aún más difícil. Admiro muchísimo al que decide criar un hijo en estas situaciones (y sí, tal vez no lo decidiste, igual te admiro).
Pediatrics
Nine weeks ago, I started my pediatrics rotation. Nine peculiar weeks, to the point that during this last week, I couldn’t even open Hive.
A madness!
I ended up in the Emergency Department of Marfán Hospital, and the situation in our city is alarming.
The hospital is going to be converted into the National Cochlear Implant Institute. In the midst of the conversion, we found ourselves involved.
The pediatrics ward, where we were supposed to rotate, was closed (due to a lack of staff, the conversion, lack of supplies—everything contributed to this decision).
The conditions in the emergency room were deplorable.
So much so that I had to walk all over the hospital to find a mattress. This was the only way to rest for a few hours in the consultation room.
Otherwise, I had to sleep on top of a bedside table (yes, just like in Surgery).
The service was limited only to very serious patients, based on absurd admission criteria.
The rest of the patients, due to the reality of the country, were referred to other institutions (quite far away) or were advised to seek monitoring from the health department.

Por favor, cuando vayas al médico...
-Lleva los medicamentos que tengas en casa, con lo que has tratado a tu niño hasta ahora.
-No lleves a tu niño a menos que tenga 72 horas de fiebre (por el nuevo protocolo, te indicarán medidas generales y tendrás que volver cuando se cumplan las 72 horas de fiebre).
Esto tiene algo de base médica y es que los valores de los resultados, en muchas de las enfermedades pediátricas, no empiezan a variar hasta cerca de las 72 horas.
-Si tu niño hace vómitos, lo primero es hacer un reposo gástrico de tres horas.
Nada de líquidos ni comida en las primeras tres horas después del vómito. Luego comienza con una cucharadita de Sales de Rehidratación (50-100 ml por deposición) o agua hervida (si la puedes filtrar, mejor aún).
Por favor, disminuye la ingesta de refrescos "de paquetico". Son puro veneno y los causantes de muchas enfermedades urinarias en los niños.
-Hoy (julio/2025) existe un brote de hepatitis, tanto infantil como en adultos. Si conoces a alguien que haya sido diagnosticado, aleja a tu niño de esa persona y extrema las medidas de conservación e higiene en el hogar y con los alimentos.
No vayas al hospital por esta enfermedad( a menos que tu niño este muy mal) Según los protocolos, el área de salud es la encargada del manejo de los casos de sospecha o diagnóstico de hepatitis.

Please, when you go to the doctor...
Bring the medications you have at home, along with what you have treated your child with so far.
Don’t take your child unless they have had a fever for 72 hours (according to the new protocol, they will give you general measures, and you will have to return when the 72 hours of fever have passed).
This has some medical basis: the values of the results in many pediatric diseases do not start to vary until about 72 hours.
If your child is vomiting, the first thing is to do gastric rest for three hours.
No liquids or food for the first three hours after vomiting. Then start with a teaspoon of Oral Rehydration Salts (50-100 ml per bowel movement) or boiled water (if you can filter it, even better).
Today (July 2025), there is an outbreak of hepatitis, both in children and adults. If you know someone who has been diagnosed, keep your child away from that person and take extreme measures of conservation and hygiene at home and with food.

A pesar de las cosas negativas y todo el trabajo que tuve que pasar, la rotación me encantó.
Al estar cortos de personal, los internos recibimos mayor autonomía.
Esto se tradujo en que tuvimos una consulta para nosotros durante las guardias.
Como paciente, te puede asustar, pero la gran mayoría estamos preparados para el manejo general del paciente.
Como estudiante, es lo mejor que te puede pasar. Por supuesto, las primeras guardias me sentía algo indeciso y confuso, porque no quieres poner un tratamiento mal, no quieres equivocarte. Con el tiempo, te das cuenta de que llevas seis años de estudiante, seis años en Cuerpo de Guardia y sabes tratar la mayoría de las patologías, casi intuitivamente.
Despite the negative things and all the work I had to go through, I loved the rotation.
Due to the staff shortage, we interns received greater autonomy.
This translated into having a consultation for ourselves during the shifts.
As a patient, it might scare you, but the vast majority of us are prepared for the general management of the patient.
As a student, it’s the best thing that can happen to you. Of course, during the first shifts, I felt a bit indecisive and confused because you don’t want to make a wrong treatment; you don’t want to make a mistake. Over time, you realize that you’ve been a student for six years, six years in the Emergency Department, and you know how to treat most pathologies almost intuitively.
Hay pocas cosas peores en este mundo que un niño enfermo. En medio de la noche hemos tenido que remitir a pacientes por falta de medicamentos; no es fácil la realidad que vivimos y a nosotros nos duele más que a nadie.
Aun así, prefiero quedarme con lo positivo.
No hay cosa mejor que la gratitud de un padre cuando le explicas el tratamiento y le aseguras que su hijo mejorará. O las gracias de un niño cuando sale de la consulta.
Pediatría me aportó mucho y hubiese querido que durara un poco más.
El niño no se parece en nada al adulto, pero las enfermedades son muy parecidas a las de Medicina Interna.
Pediatría me permitió entrar en calor para la última rotación: Medicina Interna. La vuelta a "casa" y donde además haré mi examen final (el Estatal).
No fue fácil, pero se logró.

There are few things worse in this world than a sick child. In the middle of the night, we have had to refer patients due to a lack of medications; it’s not easy, the reality we live in, and it hurts us more than anyone else.
Still, I prefer to focus on the positive. There is nothing better than the gratitude of a parent when you explain the treatment and assure them that their child will get better. Or the thanks from a child when they leave the consultation.
Pediatrics taught me a lot, and I wish it could have lasted a little longer.
A child is nothing like an adult, but the diseases are very similar to those in Internal Medicine.
Pediatrics allowed me to warm up for the last rotation: Internal Medicine. The return to "home," where I will also take my final exam (the State exam).
It wasn’t easy, but it was accomplished.
One less!
Ahora vienen las últimas nueve semanas. ¡De vuelta al Fajardo!
Nueve semanas muy intensas. Con las últimas vacaciones por medio, pero trataré de contar cómo me va.
