Importance of repeating an ECG

An expatriate visited me on Wednesday with h/o chest pain for the previous three days, fatigue, epigastric pain/fullness, and constipation. He was a diabetic, and his blood sugar wasn't under control. Before coming to me, on the previous day (Tuesday), he visited another GP doctor. His ECG was done there, and it was normal. His blood sugar was found to be high, and medicines were given accordingly.

In my chamber, he didn't have active pain. His main problems were fatigue, epigastric/abdominal fullness, and uncontrolled DM. Although I had wanted to repeat the ECG, in the end, it didn't happen. As his previous ECG was normal and he didn't have active pain at that time, like before, we concentrated on treating his high blood sugar. And, it was a mistake! I adjusted his medications and asked him to come in for a follow-up after 7 days.

The next day (Thursday), he was brought again. My duty was over already. Another GP doctor was on duty. That time, he was quite ill with shortness of breath and excessive sweating, along with chest pain. An ECG was repeated, and all the features of an acute heart attack were revealed! He was escorted to the Emergency department after initial resuscitation! If the ECG had been done on Wednesday, we could have diagnosed his heart attack earlier!

On the last workday of the week, I worked full-time in two shifts. I examined a total of 21 patients in my chamber. There was an emergency case who came with a big and nasty cut injury to one of his fingers. I referred him to a surgeon. In the visa medical section, I examined some people who came for the fitness test.

At home, after lunch, I walked on the treadmill for 10 minutes. Later, I had a short afternoon nap. In the night after 9 pm, we visited Papa John's pizza store and bought 2 medium-sized pizzas. On our way back, we stopped at a small park for a short period. My elder kid and I played badminton. The little one didn't like the park and started crying. Hence, we cut short the time there and returned home.
