Fine Needle Aspiration Procedure for Cytology

Hello everyone, how are you all? i hope you all are doing well and i am too. This post is all about medical thing. i just wants to show you how the Fine needle aspiration cytology procedure will be done.

First the patient will consult doctor complaining they had thyroid swelling or swelling in the neck region or on the body or lymph node swelling or lumps in the breast etc.... Clinicians will clinically examine it and they will send the patient to pathology department for Fine needle aspiration Cytology(FNAC). FNAC is the diagnostic test and it will tells the nature of the swelling or lesion like whether its a bening or malignant lesion or infectious or metastatic etc....

So now when we receive the patient with FNAC form which is written and filled by clinician we will cross check name age and everything and we will take history and we also again do clinical examination the size of swelling, consistency, tenderness, whether they are single or many in the body thoroughly.

After taking clinical history and examination now we will ask the patient to lie down on the procedure bed in the convenient position for us. Then we will select hypodermic needles depending upon patients age whether they comes under paediatric category or adult and type of lesion or swelling and location of swelling eg., thyroid swelling we will select needle which is having narrow bore(23 gauge, 24 Gauge,26 gauge etc) or lipomas we will go with green color needle(21 gauge).

I have told about hypodermic needles, its uses and disadvantages in my previous post and you can go through it i will share link at the end of this post. So the patient will lie down, and we have to counsell the patient and make them sure not to move while doing procedure because its a little bit painful procedure and the problem comes when we do on paediatric age group .We have to clean the area where we are going to do FNA with spirit and antiseptic and When we have to hold the swelling tightly and stretch the skin and fix it so that it won't move. Then with hypodermic needle pierce the skin and go inside the lump or swelling and do to and fro fastly so that the cells will be aspirated. Then we have to keep the cotton in that site to avoid and bleeding and fix the aspirated needle with syringe and expel the aspirated content on slide and make smears immediately and it should very fast and keep the slides in alcohol and fix it for few seconds. Now to see cellularity we have to change the slide from alcohol to toluidine blue for few seconds and then wash with water. Then we have to see under microscope and this is called ROSE(Rapid On Site Evaluation. we can do maximum of 3 times if we are not getting enough and good cellularity for diagnosis.
